RELEASE OF INFORMATION AUTHORIZATION
PROCESSING FEE AGREEMENT

TO WHOM IT MAY CONCERN:

I / We hereby authorize Cardelli Home Loans or any of its assignees to verify my / our bank accounts, credit history, employment, income and any other information which they deem necessary in connection with my / our Home Loan Application.

In addition, authorization is given to Cardelli Home Loans to pursue a loan approval with a lender of their choice for the Real Estate or Mobile Home Loan which we have applied over the phone, by mail, electronically or in person.

I/ We agree to pay a minimum of $95 processing fee for the above services rendered.

A photographic copy of this authorization and my / our signature(s) may be deemed to be the equivalent of the original and may be used as a duplicate original.
